14 year old Patient student by profession came to opd with complaint of abdominal pain since 15 days
History of present illness
A 14 year old male patient was asymptomatic 15 days back,then he noticed dragging type of pain in his abdomen which was very painful
He used few medications given by his local doctor
It didnt resolve then he came to our hospital
History of past illness:
He was suffering with same type of pain 5 months ago
No history of hypertension, diabetes, asthama, tuberculosis, thyroid.
Family history:
His sister used to get the similar type of pain
Personal history:
Apatite normal
Sleep inadequate
Mixed diet
Normal micturation
Normal bowel movements
No Addictions
No history of any drug allergy
General examination:
Patient is conscious, coherent, cooperative
No history of pallor,icterus clubbing, edema, lymphadenopathy
History of abdominal pain
VITALS:
Temperature afebrile
Bp 110/70mmhg
Pulse rate 82 bpm
Respiratory rate 16 cycles/min
SYSTEMIC EXAMINATION
CVS
No thrills ,no cardiac murmers
S1,s2 sounds heard
Respiratory system
N9 wheeze
No dyspnoea
Position of trachea is central
Abdomen
Liver nonpalpable
Spleen is palpable
Bowel sounds heard
CNS
No neck stiffness
Normal speech
PROVISIONAL DIAGNOSIS:
Splenectomy
