56YR OLD MALE WITH PAIN ABDOMEN

56yr old male came to causality with cheif complaints of pain in upper abdominal region 

HISTORY OF PRESENT ILLNESS 
Patient was apparently asymptomatic 4 days back
Then he developed abdominal pain 
With abdominal distension 
And decreased urine output 
No history of vomitings, nausea  constipation 

HISTORY OF PAST ILLNESS 
No history of hypertension 
No history of diabetes 
No history of asthama 
No tuberculosis 
No thyroid 

PERSONAL HISTORY 
Apatite normal 
Sleep inadequate 
Diet is mixed
Decreased micturation
Bowel movements regular 
No known history of allergies to drugs 
Regular Alcoholic 

NO RELEVANT FAMILY HISTORY 

GENERAL EXAMINATION 
patient is conscious  ,coherent  and cooperative 
No icterus 
No cyanosis 
No clubbing 
No lymphadenopathy 

Vitals:
Temp - afebrile 
Pr :102bpm
Rr 20cycles/min
BP 120/90 mmhg 
Spo2 98% 
Grbs 94mg

Systemic Examination 

CVS - s1 s2 +
 ,No murmurs 
No thrills

RS - No dyspnea, vomiting, vesicualr breath sounds
Position of trachea : central

Abdomen- Distended, no palpable mass
Liver not palpable
Spleen : not palpable
Bowel sounds :Yes

CNS - 
Conscious
Speech- normal
Neck stiffness- No

Investigation 

Provisional diagnosis- pain  abdomen 

Treatment
TAB. Dolo 650 mg BID
Injection. Neomol1 gm
Injection. Pan 40 mg
Injection vitamin b12 1500 mg in 100 ml NS over 1 hr
