53 YR OLD MALE WITH BILATERAL RENAL CALCULI

A 53 yr old male driver with occupation came with a chief complaint of renal calculi in both kidneys 

HISTORY OF PRESENT ILLNESS 
Patient came to causality with pain in loin to groin which was insidious onset, gradually progressing. He was asymptomatic 10 days back.
He came to our hospital  2yrs back with with compliant of renal caliculi in both kidneys..
Surgery was done before and few calculi were removed . Few months after surgery he developed same pain again

HISTORY OF PAST ILLNESS 

No h/o hypertension 
No h/o diabetes 
No h/o tuberculosis,asthma, epilepsy

PERSONAL HISTORy

 Diet - vegetarian 
Apatite is normal
Sleep is normal
Addictions  :Alcoholic 6 yrs back. But now Addictions subsided 
No known allergies

FAMILY HISTORY

No relevant family history 

GENERAL EXAMINATION 

Patient is conscious, coherent, cooperative 
Speech is normal 
No visible pallor,icterus, cyanosis,clubbing, lymphadenopathy, oedema. 

Vitals
Temperature: afebrile
Blood pressure:110/80mm Hg
Pulse rate:80bpm

SYSTEMIC EXAMINATION

CARDIOVASCULAR SYSTEM :
s1,s2 sounds are heard 

RESPIRATORY SYSTEM:
Cental position of trachea
No wheeze 
No dyspnoea 

Cental nervous system:
No neurological defects

INVESTIGATIONS 


treatment 
Tab dolo 650mg
Tab pan 40mg
Inj magnex forte 1.5g IV BD
IVF  NS 50ml
Soft diet
